Patients will receive email invitations from "Kaizen Psychiatric Services" between 9/20 - 9/22 with instructions on logging into your portal.
Please read important information to assist during this transition.
ALL PATIENT PAYMENTS, COMMUNICATION, AND TRACKING WILL OCCUR WITHIN THE NEW PORTAL BY THE ABOVE EFFECTIVE DATE.
SIGN UP PROCESS
Check your email inbox and spam folder for an invitation email titled "Patient Portal Registration" from "Kaizen Psychiatric Services".
Once logged in, click your name in the upper right-hand corner, then click "Manage Account" to review/add/edit contact information.
TIP: Under "Pharmacies", although the header says "Emergency Contact", you will still insert the Pharamcy contact information.
Please complete all requested forms and questionnaires within your patient portal.
If you have received a Credit/Debit Authorization Form, please add your credit or debit card on file for copay and balance charges. As a reminder, our office policy mandates that all private and self pay patients maintain a credit or debit card on file.
Patient medications, diagnoses, basic contact info, appointments, balances/credits, and records will be available in the new portal within 30 days of opening.
If your information is not fully uploaded within your portal, please contact our office by sending a portal message.
Patients are not expected to bring over any historic records, althoough access to the old portal will remain temporarily available.
The "Announcements" tab will show any current annoucments from our office.
The "Useful Info" tab will show current polcies, rights, and consents.
