Preparing for Your Appointment

In order to ensure an orderly and smooth start to your first appointment, we ask you to follow these quick steps below:
Read Your Welcome Email and
Acknowledgement of Privacy Practices
Within 48 hours of establishing your first scheduled appointment, you should receive a "Welcome to Kaizen Collective" email. This email will contain a reminder of your appointment date and time, how to access your appointment, contact information, as well as instructions on how to complete your other optional forms.

You must also ensure that you have received and had the opportunity to review our HIPAA form that details your rights, your choices, and our responsibilities regarding your information. We will be sending a copy of this information with your intake packet. Your signature indicates you have received these forms.

Connect to Your Telehealth Appointment
At this step, you should have done the following:

Completed and/or submitted any prior or recent relevant medical records

Now all that is left is for you to join us at your scheduled appointment time by accessing the telehealth link at the top of the page or via your phone or desktop through your appointment reminder.
